Mission Statement
Lake Forest Transition is dedicated to preparing our young adults for postsecondary success and independent living through a strength based approach that fosters inclusive learning experiences, person centered planning, community engagement, and career pathways.
What are Transition Services?
"Transition services" means a coordinated set of activities for a child with a disability that:
(i) is designed to be within a results-oriented process that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child's movement from school to post-school activities, including post-secondary education, which may include for-credit courses, career and technical education, and non-credit courses and instruction, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation;
(ii) is based on the individual child's needs, taking into account the child's strengths, preferences, and interests;
(iii) includes instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and, if appropriate, acquisition of daily living skills, benefits counseling and planning, work incentives education, and the provision of a functional vocational evaluation.

- Prepare our young adults for postsecondary success and independent living by guiding post secondary outcomes, linking community resources, and fading supports when appropriate.
- Support an inclusive learning environment where all young adults, faculty and families feel a sense of belonging, are valued, and respected.
- Utilize our young adults’ strengths, preferences, and interests in person centered planning of the 5 Transition Domains (recreation and leisure, vocational and post-secondary, executive functioning, social emotional, and independent living) to ensure career, college and life readiness.
- Foster community relationships with other Transition centers, community organizations, adult transition linkages, and/or post-secondary educational/training opportunities to support our young adults and their families in the post-secondary transition process.
- Most importantly, we believe in celebrating successes, creating lasting friendships, and having fun!
