Concussion Symptom Inventory

The Youth Sports Concussion Safety Act (eff. 8/3/2015) requires that all athletes must complete the school approved Return-to-Learn and Return-to-Play protocols, be cleared by a physician licensed to practice medicine in all its branches or a licensed athletic trainer working under such a physician (as the athletic trainers at LFHS are), and the parents have signed a Post-Concussion Consent Form.
