Health Services
Freshmen - physicals MUST be turned into the nurse’s office before the first day of school. Transfer students from other states or countries must have all required forms turned into the nurse’s office within 30 days of their first day of school at LFHS. The State of Illinois’ exclusion deadline is October 15th. Your child will not be allowed to attend school if we do not receive the physical exam form by the deadline.
